REPORT OF A CASE OF MULTIPLE EARLY CANCER OF THE STOMACH CO-EXISTING WITH A BECIGN POLYP

Abstract
This is a case report of bi-focal early stomach cancer co-existing with a benign polyp in a 66 yrs. old male. There was a sessile polyp at angulus as was seen by X-ray examination, measuring about 3.0¡¿2.5¡¿1.0cm, which was proved benign histologically. At proximal and distal adjacent sites of this polyp, there were areas of granular appearance detected on the formalin fixed specimen, measuring about 1.0¡¿0.5cm and 3.0¡¿2.3cm respectively. Serial sections of these lesions including polyp showed mucosal cancers arising from two different foci, distal one of which just invading a part of the lamina propriae mucosae of the polyp.
These superficial cancer lesions were neither recognized on the gastrocamera film nor on macroscopical examination of the fresh specimen of the resected stomach. Retrospective examination of the color slide of the fresh stomach specimen also showed the lesions to be hardly recognized. These lesions became obvious after 24 hrs. fixation in 10% formalin solution. The types of these bi-focal early cancers were ¥± a and ¥± a + ¥± b respectively.
This case is the 6th early stomach cancer endoscopically examined and surgically proved at this university hospital.
Brief discussions on multiple cancers of the stomach were done.
